Featured Partner: Association of State and Territorial Health Officials

Dedicated to supporting, equipping, and advocating for state and territorial health officials in their work of advancing health equity and optimal health for all, the Association of State and Territorial Health Officials (ASTHO) has been a key partner in advancing the Million Hearts Initiative with its members in all 50 states, 8 U.S. territories and freely associated states, and the District of Columbia, spanning 10 time zones across the globe.

The Million Hearts® Hypertension Control Champions Announced

In 2019, Million Hearts® recognized 17 solo practitioners, large health systems, community health centers, and specialty groups as 2019 Champions for their efforts to achieve blood pressure control for at least 80% of their adult patients with hypertension. Together, these dedicated professionals cared for nearly 360,000 adults where blood pressure control was achieved in 80% or more of their patients.

Toolkits Provide Resources for Community Connections

The Million Hearts Collaboration has worked with our partners to develop The Million Hearts® Collaboration Community-Clinical Linkages Toolkit to help public health practitioners implement the seven LINKAGE strategies. You can also access information and toolkits for Million Hearts® in Municipalities Project and other initiatives.

Messaging Support Materials Inspire and Promote Million Hearts®

From webinars to message support materials highlighting monthly health themes such as February’s Heart Month or September’s Cholesterol Month, the Million Hearts Collaborative compiles an easy-to-use collection of messages and resource materials.
